Echo - How To Send An Intake Form To A Patient

To send an intake form to a patient, follow the steps below:

  1. Open up the patients forms tab in Echo.
  2. On the dashboard, click on the big orange button named "Send Form to patient".
  3. Select the appropriate form from the dropdown list.
  4. Type the patient’s name and email.
  5. Optionally, you can add a note to your patient. This note will be displayed above the first question of the intake form.
  6. Click on "Send Form".

That’s it. You’ll be notified when your patient completes the form.

If you need to see the form you just sent, click on "Lists > Intakes" and search for the patient's name.

You can also send forms automatically via your booking widget, through the Secure Messaging System, through scheduled forms before/after an appointment, by adding forms to your website, or by placing a link to forms on an iPad/tablet device.
